APPLICATION FORM FOR THE DATA SUBJECT WITHIN THE SCOPE OF THE LAW ON PROTECTION OF PERSONAL DATA NO. 6698

Please fill out the following application form clearly and completely in accordance with the Law on Protection of Personal Data (“KVKK”) to submit your request to our company, La Rachel Paris Kozmetik Sanayi ve Ticaret Limited Şirketi (“Company”).

We will respond to your application as soon as possible and no later than thirty days, depending on the nature of the request. If the information and documents you provide are incomplete or unclear, we will contact you to clarify your request.

Our Company reserves the right to request additional documents and information to verify identity and authority to eliminate legal risks arising from unlawful and unjust data sharing, and especially to ensure the security of your personal data. Our Company is not liable for requests arising from incorrect information or unauthorized applications regarding the information you provide in this form. All responsibility arising from illegal, misleading, or incorrect applications lies with you. Under the Communiqué on the Principles and Procedures of Application to the Data Controller, if our response to your application exceeds ten pages, a processing fee of 1 Turkish Lira will be applied for each page after the tenth page.

Within this context, applications to our Company made in “written” form can be submitted by taking a printout of this form and delivering it:

  • In person,
  • Through a notary public,
  • By sending it to our Company's e-mail address with documents proving the applicant's identity.
